I am confused and kind of at a loss as to what I should do. I talked with the LC about the pain I was having while pumping as well as how sensitive my nipples were. She suggested that I get a smaller breast shield. I did and I have to say that it really has helped with the pain but it would seem that when I use the smaller size that my milk doesn't come out so well. So far twice today I started out with the small and then about half way through I changed the shield back to the larger one and got more milk. Should I keep doing this, stay with the small or go back to the larger shield? I think for the most part there really wasn't any pain as long as I don't start out with the larger shields.
Does anyone have any advice or have been through this?

do whatever causes the least amount of pain but gives you the most output. I hate to say it but maybe try and "tough it out" until your breasts are tougher? I know that sounds so old school (I hate that I say it) but it may be true?? Either way, good job sticking with it... it will get easier and is SO worth it. congrats!

I had to move up a shiel size, to get better output, but it didn't cause me any pain?

Have you tried using a little lanolin or oil (like olive or canola) to lube up a bit before you pump? Sometimes, that can help.

Are you using a manual or electric pump?
