Sometimes when I nurse DS will "fall off the nipple". This will usually happen when I am very full and he has a harder time latching on. What will happen is I think his latch or sucking will change because I will feel his jaws clamp down on my nipple and then my nipple will slide out of his mouth. When this happens milk is still flowing out of me and he has a mouthfull of milk. Is this OAL? This only happens when I am really full though. Sometimes it will only happen once othertimes it will happen multiple times near the beginning of a feeding.

It sounds like you're too engorged for him to get a good latch. You could nurse him sooner or hand express off just a smidge to soften the breast.

Many babies will try to clamp down on the nipple in an attempt to stem a milk flow that is too fast for them. How old is your baby? Have you tried reverse pressure softening? This link shows you how:
