I don't feel that my 12 week old drains my breast when she breast feeds. During the day she get the bottle with "mommy's milk". When I get home she is ready to eat, but my breast are never drained?

Well, I can pump 5 oz 4 times a day at work. When I get home and latch her on she is on for 5-10 min and then falls asleep. Wakes up and eats again around 8pm and then 11pm. I may be overreacting, but my breast just seem full all the time!!

That's right. They shouldn't feel full as now they release the milk on demand when the baby feeds. Pretty cool isn't it!

When the Nanny takes out the the milk from the fridge, how long can it sit out?

It should be used within a few hours if the room temperature is below 72 degrees.

Is 4 hours on the counter a good rule of thumb?
