I am making up a list of questions to ask the midwife tomorrow, it is my first appointment. What is important?

So here is my list so far... I think I have covered everything I want to know at the moment but I have a feeling I might be forgetting something.

How long have you been in practice?
When and where did you train?
Why did you decide to become a midwife?
Do you have any children? Where/how did you birth them?
How many births do you attend per month?
What percentage of your patients begin labor naturally?
What is your policy on induction?
Which hospitals do you have privledges at?
Wo do you share on call duties with? Will I get to meet them all before birthing? Can I choose not to see one if I don't feel comfortable with them?
What are your definitions of high risk?
How do you handle high risk pregnancies?
What is your experince home birthing with high risk pregnancies?
How do you feel about me having a doula present?
What routeen tests, if any, do you perform?
What can I do about the nauesa and fatigue I am experiencing at work?
Safety of ginger, coffee, honey, nuts?
I am constipated, all my fiber pills say they are unsafe. Help!
What is your experince with water birth?
Are you okay with children/other people at the birth? What about photography?
I am breastfeeding and pregnant and can barely eat. What should I be eating to keep up milk supply and sustain myself?
Water makes me gag/throw up every time I try and drink it. Obviously I need some sort of substitute...
Is the weight loss I have been experiencing okay?
How much should I be aiming to gain since I am overweight?
What exercise can I start doing? (explain about car accident)
Ask about the prenatals and which ones I should switch too.
andddd...the embarrassing one.
How do endorphin rushes affect babies and is spanking okay ( I am going to go bright red with this one )

Those all look fine. I'm not sure why you would even need to ask the last -- sexual activity is fine in a low risk pg. How could the endorphins hurt the baby? They're natural.

I think the spanking issue would be a commonsense answer. I can't see how that could affect the baby. But if it seemed like it was that hard, then just tone it back.

Unless you want to use the question to sort of introduce your intimate life to her, which you might just do more straightforwardly...

if a pregnancy is deemed high risk will you midwife continue with your care under the supervision of an ob, or will the ob take over as primary caretaker.

What, if any medications are you licenced to carry?
Pitocin for hemorrhage?
Oxygen if myself or baby is in distress?
Are you licenced to carry and perform Sutures? and do you carry the numbing meds for them?

I don't know if your planning a home birth, birthing center or a Hospital attended birth. Thought I would throw these out just incase you planning a home birth. Good luck!
