Hi, I hope you can help me. My baby and I have had what seems to be a yeast/thrush infection since her week one after cracked nipples. She is now 11 weeks. (This went undiagnosed for about 3-4 weeks until a crisis point of unbearable pain.) We have been through Nystatin 2 separate rounds (about 40 total days) with a symptom free week between and a week of symptom elevation. And 24 days of dyflucan with a symptom free week and then a second week of building pain before round 2 (the last 10 days of meds.) I have been off the dyflucan since 10-23 and had a great week of no pain, no symptoms and this last week began experiencing pain in my milk ducts like shooting stars before experiencing the nipple pain in the last day or so that escalates to the point of excrutiating making it impossible to nurse. THis is what happened last time to. This is now my third bout with this.



My Dr sent my breast milk to be cultured and it came back "no yeast" and my pediatrician sent my baby's diaper off to be cultured and it came back "normal". (She had terrible tummy and gas type pain as well as other behavioral signs of yeast and with the second reccurance had the white spots in her mouth. I happen to think it had gotten into her intestinal system because the behaviors and the pains all resolved within a week of beginning this last round of nystatin.) She had been having intensely dark green runny, mucusy stools that now have become more yellow-green in color, sometimes almost neon but continue to be runny and mucusy despite both of us not being on meds.

I am adminstering probiotics for both of us but admittedly have not been diligent with it since the resolution of symptoms. I continue to use Monostat 7 at times in her diaper area and wash everything used daily. Behaviorly she has gone from a crying, gassy, "colicky", hard to console baby who is always popping off the nipple and refusing to nurse past the first let down to a calm, smiley, mostly "easy" baby and nursing much better within a few days of beginning the last nystatin treatment.

Do you have any advice whatsoever? I have Dr. appt but know that Drs. aren't experts in breastfeeding. Please help, we cannot continue to deal with this over and over.

THis is a difficult situation for you and a difficult question. THe latest research is questioning whether yeast is or has ever been the cause of these types of problems. Dr. Thomas Hale and others are not treating these probelms with antifungals. We are now looking at the nipple damage and nerve damage as being the cause of the deep breast pain due to vasospam. The reason the antifungals can give some relief is because they have some antibiotic properties. 98% of cultured "sore" nipples grow out baceria, not yeast. So what is the answer? If you are local I would say I need to look at your baby feed to determine the root cause of the problem. The other point is that yeast if it does occur, doesn't occur in the first 3 weeks. I can't really offer you more without laying eyes on your baby feeding and your nipples.

Thanks, debbie for your prompt reply. The milk and stool were cultured for bacteria too. And my baby's latch could be improved I think, though we go for long periods of time with no nipple pain, no symptoms with her latch remaining the same. I am in California and have been to lactation consultants who seemed to think her latch was fine. Mostly round nipples most of the time, though one nipple she does like to squeeze a bit and I end up with a slight white line. I think it's called a lipstick line?
I will pursue these other options though am uncertain how to obtain the APNO and grapeseed extract combo. Can these be easily obtained at a pharmacy?

Also, I did have symptoms consistent with vasospasm in the beginning which recurred when the pain was intense but not during treatment or after.

APNO is by prescription. Grapeseed extract you can get at any health food store.

Oh yes, thanks for reminding me. Are you producing lots of milk? Does your baby ever choke and sputter?
